Caine is a blind assassin who worked for the High Table and retired to live near his daughter. He's an old friend of John Wick from the days when they both killed for the same organization. He fights by ear as if he could see.
After John kills the Elder, the Marquis Vincent de Gramont tracks Caine down in Paris. His daughter, Mia, is playing violin in a park. The Marquis makes it clear that if Caine won't take the contract on John, she's the one who dies. Caine takes it.
Caine arrives at the Osaka Continental with Chidi, the Marquis's right hand, and the High Table's soldiers, hunting John. The manager, Shimazu Koji, is an old friend of both men. Koji evacuates the hotel before the attack.
To make up for his blindness, Caine sets out motion-sensor doorbells that chime when someone gets close. Koji won't let him through to John, and the two friends square off. Caine kills Koji but spares his daughter, Akira.
In Berlin, Caine follows John to Killa's club, where John plays cards to win back his place in the Ruska Roma. In the chaos that follows, Caine and bounty hunter Mr. Nobody hold off the thugs.
In Paris, Caine and John talk in a church about what each of them stands to lose. The Marquis picks Caine to fight for him in the duel set for sunrise at Sacré-Cœur. On the climb up Montmartre's 222 steps, surrounded by assassins, John gets help from Caine to make it in time.
The duel is fought with antique pistols, three rounds. In the first two, both men are hit. In the third, Caine shoots John squarely. The eager Marquis takes Caine's gun to fire the final shot himself, and John uses the bullet he held back to kill him.
The High Table's representative declares Caine and his daughter free. In the post-credits scene, Caine returns to Paris with flowers to see Mia. On the way, Akira appears with a knife, out for revenge for her father.
Caine is played by Donnie Yen, who helped shape the character. In 2025, Lionsgate greenlit a solo film, Caine, directed by Yen and written by Robert Askins and Mattson Tomlin from a pitch by Yen and Chad Stahelski. Filming started in April 2026 in Budapest with a Hong Kong leg planned. No release date has been announced.
